"Yellow Dot " Program For Illinois Residents

Illinois Departments of Public Health and Transportation Divsion of Traffic Safety, have partnered to offer a life-sa ing program called "Yellow Dot" designed to provide vital meical information on vehicle drivers/passengers in the event of an emergency.

Vehicles identified by the yellow decal on thd rear windshield will alert first resonders (e.g. fire fightes, paramedics, police officers) that a yellow medical card containing vital personal and medical information is in the vehicle's glove compartment.

Materials are free to all Illinois citizens and include a pamphlet, yellow dot decal, and yellow medical information card to provide current photo and personal information (name, emergency contact, personal physician, medical conditions, recent surgeries, allergies and current medications).

Residents can pick up these materials at Mchenry County Department of Health's (MDH) offices in Woodstocak --2200 N. Seminary Avenue-- and at the Crystal Lake office --100 N. Virginia Street
